What a personal passport is
The personal passport is an information-sharing tool created as part of Camden’s Homelessness System programme. It was co-produced with a group of people who have personal experience and knowledge of homelessness. In their words, the personal passport aims to:
- "help workers understand you as a person overall, not just about your circumstances...to help reduce having to explain your issues over and over"
- "contain information that can be used to help solve problems and personal situations without having to repeat the same story many times that can be upsetting or just annoying"
The passport is available as a downloadable, interactive PDF that can be password-protected.
In response to user feedback, we've created 2 versions of the passport. The shorter version may be more appropriate for people who find open-ended questions too overwhelming.
By completing a personal passport, people can speak for themselves and have greater choice and control over how they are supported and understood.

When to use it
The personal passport can be completed:
- as part of a professional referral
- during an assessment
- to support relationship building and understanding
This tool is an example of trauma-informed practice.
